Advance Care Planning (ACP) is for individuals of all ages and health conditions. It is a voluntary discussion to articulate your healthcare preferences to both your loved ones and healthcare providers. Additionally, you will be invited to designate a Nominated Healthcare Spokesperson (NHS) who can assist in making crucial decisions in the event that you become unable to communicate.
The outcomes of the ACP discussion will be documented in the form of an advance care plan. This plan serves as a guide for your healthcare team and loved ones when making decisions on your behalf during emergencies. Conducting these conversations in advance provides your loved ones with peace of mind, eliminating assumptions about your preferences in a medical crisis. The advance care plan facilitates informed decision-making that aligns with your life values and healthcare goals during critical periods.
The ACP programme is supported by the Agency of Integrated Care (AIC), whose approach is adapted from the Respecting Choices® framework (Gundersen Health System, USA).
NUH initiated ACP in late 2011 with three disciplines: Cardiology, General Medicine/Geriatric and Oncology. Since then, the programme has expanded and now includes eight disciplines:
- Cardiology
- General Medicine / Geriatric
- Infectious Diseases
- Liver
- Renal
- Respiratory
- Oncology
- Neurology
- Rheumatology

Predicting the course of our health is beyond our control, and entrusting our family with decisions during a medical crisis can be a source of stress. Hence, we should communicate our wishes and preferences to our loved ones while we are able. This proactive approach ensures that they are not left to make assumptions about your desires in the midst of a medical crisis.
