Diabetes, as a chronic condition, requires effective self-management.
The Chronic Disease Self-Management Programme (CDSMP) offers interactive online aimed at empowering individuals to actively manage their diabetes. These sessions foster mutual support and enhance confidence in handling health challenges, promoting active and fulfilling lives.
Who is the programme for?
The programme welcomes anyone managing one or more chronic conditions, such as diabetes, obesity, hypertension, asthma, chronic obstructive lung diseases, arthritis, stroke, and more. Caregivers are also encouraged to participate alongside their loved ones.
- Duration: Six weekly sessions, each lasting two hours
- Mode: In-person sessions. Participants are encouraged to register with a family member or carer for technical support during the programme.
- Content: The programme covers coping with frustration, fatigue, pain and isolation; appropriate exercises for maintaining and improving strength, flexibility and endurance; medication management; effective communication with family, friends and healthcare providers; nutrition; informed decision-making; goal setting and problem solving.
