We are integral members of a multi-disciplinary diabetes team, aiming to care for and empower patients with diabetes through the following services:
Diabetes in Pregnancy
Led by an Advanced Practice Nurse (APN) trained in both midwifery and diabetes management, along with certified Diabetes Nurse Educators (DNEs), we offer individual and group diabetes tele-consultations and close continuous tele-monitoring follow-ups.
Type 1 Diabetes Service
Our dedicated Type 1 diabetes team, comprising an APN, DNEs, an endocrinologist, dietitians and a psychologist, offers a wide range of diabetes services. We educate patients on self-managing their condition, including titrating their required insulin to the amount of carbohydrate they consume. We manage diabetic care using technology such as continuous glucose monitors and insulin pump therapy. Additionally, the Type 1 diabetes team conducts annual group diabetes workshops to support and educate individuals with Type 1 diabetes.
Post-surgery diabetes Service
Ensuring continuity of care from inpatient to outpatient settings for patients post Coronary Artery Bypass Graft, this service aims to optimise blood sugar control and improve surgical outcomes, including shorter hospital stays and the prevention of high blood sugar and surgical wound breakdown.
Ramadan Service
We equip our Malay diabetic patients with essential skills and knowledge to stabilise their diabetes during the Ramadan period. This service involves group teaching, tele-consultations and tele-monitoring.
Inpatient Diabetes Service
Collaborating with the diabetes team, the APNs manage inpatient referrals, reviewing and caring for patients with diabetes in the wards. The focus lies in optimising diabetes treatment, offering diabetes education and continuing tele-monitoring upon discharge.
